PARTICIPANT WAIVER AND RELEASE OF LIABILITY
Please read carefully before signing.
I, the undersigned, acknowledge and agree that:
Assumption of Risk: I understand that participating in activities involving tents, décor, and other event equipment involves certain risks, including but not limited to tripping, falling, allergic reactions, or other injuries. I voluntarily assume all risks associated with participation.
Supervision: I understand that adult supervision is required at all times during the event, and that Teens Tots Cozy Nook is not responsible for supervising children or ensuring their safety during the event.
Release of Liability:
I hereby release, waive, and discharge, Teens Tots Cozy Nook its owners, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur during or after the event.
Damage to Equipment: I agree to be financially responsible for any damage to tents, décor, or other rental equipment beyond normal wear and tear.
Health & Safety: I confirm that all participants are in good health and have no medical conditions that would make participation unsafe. I will notify Teens Tots Cozy Nook of any allergies or sensitivities in advance.
Photography & Media (Optional): I give permission for Teens Tots Cozy Nook to take photos or videos during the event for marketing purposes. (Yes/No)
Acknowledgment: I have read and understand this waiver and release of liability, and I voluntarily agree to its terms.
